Work Permit Procedures

PLEASE CALL 508-376-7000 for permits outside of school hours.

For Millis High School Students:

~Print out the Work Permit Application (available here and below) ~Student should complete Work Permit Information Sheet (page 2) ~Employer should fill out and sign Employment Permit Application (page 3) ~The student and a parent/guardian must sign (page 4 - #4 and #5) ~Students under 16 years old, the application will require either a doctor's signature or you must include a copy of their current physical (page 4) ~Proof of date of birth is required. This could be a Birth Certificate, Passport, Driver's License/Permit Bring completed application and proof of date of birth to the High School Main Office or Superintendent's Office

For Non-Millis High School Students:

~Print out the Work Permit Application

~Student should complete Work Permit Information Sheet (page 2)

~Employer should fill out and sign Employment Permit Application (page 3)

~The student and a parent/guardian must sign (page 4 - #4 and #5)

~Students under 16 years old, the application will require either a doctor's signature or you must include a copy of their current physical (page 4)

~Current school must complete School Record (page 5) most recent report card can be submitted.

~Proof of date of birth is required. This could be a Birth Certificate, Passport, Driver's License/Permit
Bring completed application and proof of date of birth to either the High School Main Office or Superintendent's Office. Students not enrolled in Millis schools should call 508-376-7000 to schedule an appointment for Work Permits.
Online submission of the application can be sent to [email protected]. Please note that this requires a call to 508-376-7000 to confirm that all documents required have been received. Once we confirm that the information required is accepted the permit will be emailed within 48 hours.
